Regular Application

You will need to complete the entire application and complete the non-refundable credit card payment for your application to be evaluated. Additionally, you will need to mail all documentation as described in the instructions.

All fields marked with:  * are required. You can enter NA if not applicable.

The fees are as follows:

• $2.50 – Convenience fee for online application
• $217.00 – Total fee to process a 2-year License
• $242.00 – Total fee to process a 2-year License, and an optional wall certificate($25.00 fee)

Once the application is completed you will receive a username and password and will be able to log into your account to check the status of your application and license.
